This form may be used by teachers/staff to request reimbursement/payment for classroom supplies with high quality pictures of receipts or scanned receipts. Each teacher/staff member will receive up to a $100 reimbursement of supplies for the 2020-2021 school year. Starting November 1st, 2020 all submissions must be made within 45 days of receipt date. Reimbursements will be processed on the first business day of each month.
